Web surveys and forms are a popular way of gathering data, and Quask's FormArtist provides a cheap way to create professional looking results.
Simply design your form, upload it to your web server, and then ask participants to complete the form. Response data is emailed back to you, and you can display it in the form of an Opinion Meter on your website.
The free edition is ideal for designing forms and polls that can pop up or be embedded into a web page. It doesn't, however, have any database management capabilities, and responses are sent back by email only. If you require database functionality and want to export, analyse or report on your data, you will need to look at the Standard, Professional or Enterprise editions.

Once you have selected a page style from the supplied libraries, you can populate the form page(s) with a wide range of objects, including text input boxes, check boxes, drop-down lists and emoticons. This latter proprietary object is a five-point slider with an animated icon.
Advanced capabilities include the ability to assign a grid to a picture so that respondents can mark a photograph.
Deploying the finished form to a web server is easy, but the server must be configured to support PHP or ASP scripting. This requires getting your hands dirty.
